Let’s get your assets protected! We will begin by asking several important questions:

1. Have you previously created a will or trust and/or has someone prepared one on your behalf?If so, where is that document housed?(Required)

2. What are your goals passing on your assets to your beneficiaries? Please list any specific legacy related items which matter most to you.(Required)

3. Which individuals will be impacted by the creation of your will or trust (first and last names, along with current contact information)?(Required)

4. List your most valuable assets (i.e. heirlooms, jewelry, handbags, artwork, furniture, vehicles, real estate, etc.) If any of these assets have been promised to any heirs, please also list their full names and contact information here.(Required)

5. Are there additional individuals, companies, organizations, religious groups, etc. who you would like to leave inheritance to aside from named heirs?(Required)

6. Which investment, savings, and/or retirement accounts will be included in your estate? Please list company names and account numbers.(Required)

7. Are there any outstanding loans or debts that must be paid prior to the settlement and disbursement of you estate assets? If so, please list them below.(Required)

8. Who you would like to name as executor of your estate? Full name and current contact information please.(Required)

9. Do you have minor children/dependents? If so, who would you like to name as guardians for them? Please also list specific dependents which you would like to leave money in trust for.(Required)

10. Who would you like to act as your designated Power of Attorney for your health and finances? If not the same person, please specify each.(Required)

11. Where are your estate related documents and files stored and how can access be gained to them once you pass away?(Required)

12. Provide current contact information for your tax professional/accountant.(Required)

13. Do you own a business or multiple businesses? If so, do you have a transition plan in place?(Required)

14. Do you have pets? If so, who would you like to take care of them?(Required)
